Dear Patient,
Welcome to Mahoney Pediatrics Patient-Centered Medical Home program. The PCMH program was developed to provide comprehensive primary care that encourages partnership between patients and their physicians. As a recognized PCMH Program by the National Committee for Quality Assurance, we practice evidenced based care using the APP and Bright Futures guidelines.
Expert Healthcare
We will work with other health care providers to create a team centered care that is focused on you! As your care team, we will involve you in decisions about your health and health care to develop a stronger relationship with you. You will also have easier access to your provider through same day appointments and phone visits (when applicable).
It will take a partnership between you and us for the PCMH program to be successful. We will be discussing some changes at your next visit. If you have any questions please contact our staff. One of our providers is always available to our patients after hours by phone, for emergencies only. Please see policy page for specifics.
